Unnamed construction worker who plummeted 42 stories to his death Monday when a section of the top two floors at the Trump SoHo construction site collapsed, officials said.

Before building collapsed, co-worker snapped shot of victim (left).

A construction worker plummeted 42 stories to his death Monday and three others were injured when a section of the top two floors at the Trump SoHo construction site collapsed, officials said.
"There was a rush of concrete," said 54-year-old Walter Brown, who was working on the 41st floor of the hotel/condominium project. "Then, everything came down. ... One guy missed the net and went over and literally fell to his death."
The victim, whose name was not immediately released, was working on the top floor at the building's outer edge and missed the safety net two stories below him, officials said. He landed in an alleyway behind the building at 246 Spring St.
Another worker, Francesco Palizzotto, fell into a net and was rescued by firefighters who were lowered to him in a debris bucket by a crane, officials said.
Palizzotto was seriously injured when wet concrete fell on him while he was tangled in the net. Two other workers suffered minor injuries.
The company in charge of the site, Bovis Lend Lease, was also the safety manager for construction at the Deutsche Bank building, where violations led to a fire that killed two firefighters last year.
The Daily News has reported that the company's most prominent projects, from the Time Warner Center at Columbus Circle to Scotland's Parliament building, had a history of worker injuries and deaths and objects hitting passersby.
NYPD Assistant Chief Thomas Galvin said the workers were pouring concrete into forms when a studio-apartment-sized section collapsed. The cause of the collapse is being investigated.
"They were pouring it on the floor ... tamping out the air bubbles, and for some reasons the forms collapsed," he said.
Galvin said there was a 20-foot long, 20-foot wide hole between the 40th and 42nd floors. Forms are usually wooden panels used to keep concrete in place until it hardens.
Buildings Commissioner Patricia Lancaster said the tower was in no further danger of collapse, but she issued a stop-work order and vacated the top two floors of neighboring commercial properties on Sixth Ave.
"This is clearly unacceptable," Lancaster said, noting that safety regulations were in place to prevent such disasters.
Her department issued four safety violations to Bovis as a result of the accident.
The Buildings Department has issued 11 other violations at the site against Bovis and its subcontractor, Difama Concrete, since work began in May 2007, records show.

Those violations included failure to notify the city of an accident and failure to close a sidewalk before lifting equipment above it. Difama and Bovis were both cited in November for "operation of a crane in an unsafe manner" at the site, records show.
SoHo residents, many of whom were against the project, have complained frequently that it was being rushed and was unsafe.
In a statement, Bovis officials said the company was conducting its own investigation and working with authorities to determine the cause.
Palizzotto, an Italian immigrant from Brooklyn, was in serious condition at St. Vincent's Hospital Manhattan, officials and family members said last night.
"A lot of concrete fell on him and I can't get a response out of him," said a man who identified himself as Palizzotto's brother-in-law but refused to provide his name. "He's a good guy, a working guy."
"Construction projects are being pushed to finish at the risk to the safety of not only the workers," Manhattan Borough President Scott Stringer said.
The accident followed two other recent incidents involving Manhattan buildings.
An architect was injured at a construction site last month when debris fell onto the trailer where he was working in the shadow of a planned 45-story building at 200 Murray St. Robert Woo, 39, was not able to move his legs from the knees down after the accident.
Also last month, Alcides Moreno, 37, survived a 47-story fall while washing windows at the Solow Residences on E. 66th St. with his 30-year-old brother, who died after cables connecting the two to the building failed. With Michael White
